A request to conclude an agreement for the management of an Employee Capital Plan (PPK)
Dear Sirs or Madams,
our information shows that you have not concluded an agreement for the management of an Employee Capital Plan (PPK). This is an obligation provided for in the law1. That is why, you are requested to:
1. conclude an agreement for the management of an Employee Capital Plan (PPK) with a target-date fund, managed by a designated financial institution, i.e. Polski Fundusz Rozwoju Towarzystwo Funduszy Inwestycyjnych S.A. (PFR TFI) or to submit to PFR – using an online form – information about the conclusion of an agreement for the management of an Employee Capital Plan (PPK) with another financial institution. The time limit to be observed in this case is 30 days from the date of receipt of this request;
2. log on to the website wezwania-ppk.pfr.pl using your Tax Identification Number NIP and TOKEN, fill in the form and send it to PFR (this will take approx. 5-7 minutes).

Important! An employing entity that fails to conclude an agreement for the management of an Employee Capital Plan (PPK) may be fined up to 1.5% of that employer’s remuneration budget in the financial year which immediately precedes failure to meet this obligation2. In addition, a notification shall be sent to the National Labour Inspectorate.
Who does not conclude an agreement for the management of an Employee Capital Plan (PPK)
The following entities do not conclude an agreement for the management of an Employee Capital Plan (PPK):
- an employing entity with the status of a micro-entrepreneur3, to which all the employees submitted a declaration on resignation from making payments into the Employee Capital Plan (PPK)4,
- an employing entity which is a natural person and employs, to the extent not connected with this entity’s business activity, a natural person, to the extent not connected with this person’s business activity5.
Important! If you are one of such entities, you are also advised to respond to the request and fill in the form at PFR website. In the form please submit to PFR a declaration on meeting the conditions under which you are an entity exempt from the need to execute an agreement for the management of an Employee Capital Plan (PPK)6.
Employing entities which do not need to conclude an agreement for the management of an Employee Capital Plan (PPK) also include entities running an Employee Pension Scheme (PPE) that meets the conditions specified by law7.
Important! In such a situation, you are also advised to fill in the form at the PFR website and thus submit in this manner a declaration on running the Employee Pension Scheme (PPE), indicating the amount of basic contribution charged and paid to the PPE as specified by the Act on the Employee Pension Schemes of 20 April 2004 along with the percentage share of the employees participating in the PPE 8.

An agreement for the management of an Employee Capital Plan (PPK) may be concluded with PFR TFI in an electronic format, using the form: umowappk.pfrtfi.pl/#wniosekPPK.

Please be reminded that when the agreement for the management of an Employee Capital Plan (PPK) is concluded, you are also obliged to conclude an agreement for running the Employee Capital Plan (PPK) for the employees9.
